The Social Services Director (SSD) admitted to only providing bed hold information at the time of admission and was unaware of the requirement to do so upon each discharge for hospital or therapeutic leave. An LPN expressed unfamiliarity with the bed hold policy, assuming it was the responsibility of the SSD. The facility administrator believed that the discharging nurse was responsible for completing the bed hold notification and stated that staff had been in-serviced on this process, but was unaware that it was not being consistently implemented.
