The review of employee education files for CNAs O, Y, AA, and BB revealed that their files did not include a general orientation checklist or documentation of attendance at the facility's general orientation. Additionally, their education records did not show a minimum of 12 hours of training or competencies identified in the facility assessment. The skills fair attendance lists did not include signatures from these CNAs, indicating they did not attend the required training sessions. Interviews with staff members, including CNAs and the Director of Nursing (DON), further highlighted the deficiencies in the training program. CNAs reported not receiving training on mechanical lifts, and the DON admitted to not having documentation of training sessions. The DON, who had been in the position for three and a half months, was responsible for ensuring staff training but lacked individual records to track the required 12 hours of CNA education. The facility's previous corporate team, which was responsible for providing education, had been removed two weeks prior to the survey, contributing to the lack of training documentation.
