Summary

The facility failed to develop and implement appropriate plans of action to correct and ensure ongoing compliance with identified quality deficiencies. This deficiency was observed through a review of the facility's Quality Assurance and Performance Improvement (QAPI) Team Meeting Notes, which indicated ongoing monitoring and audits for complaint surveys conducted between June and August 2024.
