Summary

The facility's policy required immediate reporting of suspected abuse to the Administrator and Director of Nursing, but this protocol was not followed. The Administrator acknowledged that the allegations should have been reported to the state agency and that Staff U should have been immediately separated from resident care, which did not occur.
