Additionally, the facility failed to initiate and adjust Lasix medication orders for Resident #7 in a timely manner. The August Medication Administration Record showed that the staff did not start the additional Lasix doses as ordered and continued incorrect dosing beyond the specified dates. These errors were not identified by the staff, indicating a lapse in medication management and adherence to professional standards of practice.
