Summary

The facility failed to ensure that residents were free from significant medication errors by not correctly identifying a resident prior to medication administration. This incident involved a resident who was mistakenly given another resident's medications due to a room change that was not properly communicated. The Director of Nursing was training a new Licensed Practical Nurse, who administered the wrong medications to the resident because the nameplate on the door had not been updated to reflect the room change. The resident who received the incorrect medications was admitted to the facility with multiple medical conditions, including Congestive Heart Failure, Atrial Fibrillation, and Type 2 Diabetes. The error resulted in the resident being sent to the emergency room for treatment of an accidental medication overdose.
