Summary

The facility failed to ensure a thorough assessment of wounds and proper notification to the physician after a change in condition was identified for a resident. The resident, who was admitted with multiple diagnoses including a fracture, glaucoma, hypertension, heart failure, COVID-19, dementia, and a urinary tract infection, was at moderate risk for developing pressure injuries as indicated by a Braden Score of 13.
