Further interviews with the facility's staff, including the RN/UM, DON, and ADON, revealed inconsistencies in the understanding and implementation of EBP. The RN/UM was not aware that R10 was on EBP, despite the resident's pressure injuries, and there was no clear communication or documentation system in place to inform staff of residents requiring EBP. The DON and ADON acknowledged the deficiencies observed by the surveyor, including the absence of EBP signage and PPE carts, as well as the failure of staff to wear gowns and perform proper hand hygiene during care activities.
