The facility failed to establish an effective infection prevention and control program, as evidenced by the improper use of Personal Protective Equipment (PPE) and inadequate hand hygiene practices by a Registered Nurse (RN) during the treatment of a resident. The resident, who was under Enhanced Barrier Precautions due to conditions including Type 2 Diabetes Mellitus with Diabetic Neuropathy and Peripheral Vascular Disease, required specific infection control measures during wound care. However, the RN did not secure the gown properly, leaving it open in the back, and repeatedly allowed it to fall off the shoulders during the procedure, compromising the protective barrier.
