Summary

The facility failed to document a resident transfer in the medical record when a resident was transferred to the hospital due to a change in condition. An interview with the Director of Nursing confirmed that there was no information in the medical record explaining the hospitalization, and it was expected that staff should document the reason for a resident's transfer to the hospital and complete an assessment of their condition. This lack of documentation affected one of the three residents reviewed for hospitalization, with the facility census being 39.
