Failure to Revise Care Plan Leads to Pressure Injuries

Summary

The facility failed to review and revise a resident's care plan addressing pressure ulcer risk, which led to the development of pressure injuries. The resident, who had severe cognitive impairment and multiple health conditions including anemia, diabetes, heart failure, kidney disease, a thyroid disorder, high blood pressure, and dementia, was readmitted to the facility from the hospital.
