The facility failed to consistently implement assessed and care-planned interventions for preventative skin care for a resident identified as R31, who was at risk for developing pressure ulcers. Despite this policy, the facility did not adhere to the care plan for R31, leading to a deficiency in providing appropriate pressure ulcer care and prevention.
