The facility failed to serve food at an appropriate temperature and in a palatable manner during two observed meals. During the breakfast meal service, a resident reported that the food was not hot, and a temperature check of a test tray revealed that the oatmeal, french toast, and sausage were at approximately 125 degrees Fahrenheit. The facility had been using Health Care Services (H.C.S.) to manage the dietary department, but there was no dietary manager present at the time.
