Summary

The facility failed to protect food from contamination during meal service, as observed during a lunch service. Staff A, a cook, was seen handling ham salad sandwiches and French fries with gloved hands that had touched various surfaces, including the outside of a hamburger bun bag, the counter, a scoop handle, tong handle, serving lids, and her cheek.
