Summary

The facility failed to revise a person-centered comprehensive care plan for a resident regarding their advanced directives. The resident, who was admitted with diagnoses including diabetes, hyperlipidemia, malnutrition, and depression, had a severe cognitive impairment as indicated by a BIMS score of 3. Despite having a Do Not Resuscitate (DNR) form signed by the primary physician, a concurring physician, an appointed health care agent, and a relative, the resident's care plan and physician's orders continued to reflect a full code status.
