Summary

The facility failed to ensure that their licensed staff and CNAs received annual competency training, as evidenced by a review of the competency training binder which showed that the majority of staff had not completed their competency signoffs. The Staff Development nurse, who has been in the position since April 2023, reported that no training or competency records were handed over to her when she assumed the role.
