The facility failed to prevent a significant medication error for a resident with a history of hypertension and atrial fibrillation. The resident was prescribed midodrine hydrochloride to be held if their systolic blood pressure (SBP) exceeded 130 mmHg, and losartan potassium to be administered only if the SBP was greater than 130 mmHg. However, the facility staff did not adhere to these physician orders, resulting in the administration of midodrine when the resident's SBP was above the specified threshold and losartan when the SBP was below the threshold.
