The Nursing Home Administrator confirmed that transfer notices were not completed, and the Social Service Director acknowledged that they had not been submitting the required monthly notifications to the Ombudsman. This lack of adherence to policy resulted in the failure to properly notify residents and their representatives of transfers, as well as the failure to inform the Ombudsman of such events.
