The facility's 'Notification of Changes Policy' mandates immediate notification of changes in a resident's condition to the resident, their representative, and the attending physician. However, this policy was not followed for both R3 and R12. The facility failed to update the care plans and communicate changes to the care team, resulting in a lack of continuity of care and delayed treatment for both residents. These deficiencies highlight the facility's failure to adhere to its own policies and procedures, leading to adverse outcomes for the residents involved.
