This oversight involved a resident who was admitted with multiple diagnoses, including adult failure to thrive, chronic respiratory failure, thrombocytopenia, a history of cancer, heart disease, depression, and anxiety. This failure placed the resident at risk of not receiving necessary end-of-life support.
