Staff members were unaware of any assessments or orders for the restraint, and the family member did not request the chair but was informed it would help reduce falls. The facility's failure to document and assess the use of the restraint, along with the lack of a care plan and monitoring, contributed to the deficiency and the harm experienced by the resident.
