Summary

The facility failed to maintain sufficient nurse staffing levels on one of its units, known as The Mountain Unit, on the night of February 17, 2024. This deficiency resulted in a situation where a resident, who had severe cognitive impairment and required substantial assistance for daily activities, experienced a fall that went undetected by the staff for an unspecified amount of time. The resident had previously been transported to a hospital due to a fall with related injuries, and a new intervention of a fall mat was implemented. The facility's inability to maintain safe staffing levels placed the resident in an immediate jeopardy situation, with the potential to impact all 78 residents of the facility.
