The facility's policy on care plans requires that they include measurable objectives and timetables to meet the resident's needs and reflect current standards of practice. However, the care plans for both residents 23 and 10 were not updated to include significant changes in their care and activities, leading to deficiencies in meeting their current needs. The interdisciplinary team responsible for reviewing and updating care plans did not ensure that these updates were made, resulting in a failure to provide comprehensive and individualized care for these residents.
