The facility Administrator confirmed the unacceptable conditions and acknowledged the lack of evidence for routine cleaning. The Administrator also confirmed that the Dietary Manager had not completed the required training and was frequently involved in cooking, which hindered the maintenance of the kitchen environment and equipment.
