Summary

Despite being cognitively intact, the resident's needs were not met, leading to a decline in her physical condition, including the reopening of a surgical wound and the development of an open area on her bottom. The facility's failure to provide trauma-informed care and address the resident's PTSD triggers resulted in a deficiency that could potentially lead to re-traumatization.
