Residents experienced long wait times for call lights to be answered and insufficient assistance with Activities of Daily Living (ADLs), leading to issues such as skin breakdown and compromised dignity. Charting for fluid and meal intake was inconsistent, increasing the risk of dehydration and nutritional concerns. Despite these staffing issues, the facility continued to accept new residents, exacerbating the problem.
