These evaluations were not completed on 2/02/26 because the resident was sent to the hospital that day for a change in mental status. The DON confirmed that the resident had orders dated 1/16/26 for OT, PT, and speech therapy evaluations, which were acknowledged by the primary care physician on 1/21/26, and that these ordered evaluations were not completed as directed by the physician.

The PT and Director of Rehab acknowledged that the resident would benefit from therapy, that services had been discontinued due to insurance denial, that Part B coverage had not been verified, and that the facility did not have restorative programs, despite a facility policy stating that therapy services are to help residents reach maximum functional performance and transition to restorative nursing when appropriate.
