The Director of Nursing acknowledged that the incident should have been reported and expressed uncertainty about how it was missed, despite other similar incidents being reported and investigated. The Social Services Director confirmed that both residents were unable to consent to sexual contact and that the incident should have been reported as an allegation of sexual abuse.
