Interviews with facility staff revealed a lack of documentation regarding the provision of this information. On May 17, 2024, the Infection Preventionist, Staff C, confirmed the absence of documentation for Residents 3 and 20. Additionally, on May 20, 2024, the Assistant Director of Nursing, Staff B, stated that it was expected that residents be informed of the risks and benefits of pneumonia vaccinations and that there should be documentation of whether a resident received the vaccination or declined it. This oversight placed the residents and/or their representatives at risk of not being fully informed before making decisions about their pneumonia immunizations.
