Summary

The facility failed to provide appropriate treatment and care according to professional standards for two residents, leading to significant deficiencies. Resident ID #2, who had dementia and type two diabetes mellitus, experienced two unresponsive incidents that were not properly addressed. On both occasions, the provider was not notified, and no interventions were implemented. The Nurse Practitioner was unaware of these incidents and indicated that an assessment and lab orders would have been conducted if informed. The facility staff failed to alert the on-call provider when the Nurse Practitioner did not return the call, leading to a lack of appropriate response to the resident's change in condition.
