The facility's policies on safe lifting and movement of residents, as well as elopement and wandering, were not adequately followed. The policy on safe lifting did not specify when gait belts should be used, and the wandering policy did not address maintaining safety for wandering residents within the facility. The Director of Nursing acknowledged the deficiencies, noting that staff failed to communicate effectively and did not intervene appropriately to safeguard the residents involved.
