Summary
The facility, with a census of 93 residents, failed to ensure that all direct care staff received the required training on abuse, neglect, and exploitation (ANE). This deficiency was identified through a review of training records and interviews. Specifically, the credentialing files for two agency Certified Nurses Aides (CNAs), referred to as CNA QQ and CNA RR, lacked evidence of completed ANE training. During an interview, Administrative Nurse D revealed uncertainty about who was responsible for verifying that agency staff had completed the required training. Although some contracted agency staff training could be viewed online, there was no clear process for verifying and retaining training information for all agency providers. The facility's Staff Competency policy, dated June 2023, required all nurse aide personnel to participate in regularly scheduled in-service training classes, including ANE training. However, the facility did not ensure the completion of this training for all staff providing care, leading to the identified deficiency.
