The facility failed to maintain a sanitary kitchen environment, as evidenced by multiple observations of unclean equipment and improper food storage practices. During an initial kitchen tour, surveyors noted yellow splatters in the microwave, a crusty substance on the standing mixer, and significant frost buildup in the milk cooler. The Executive Director of Senior Operations acknowledged the absence of documentation for cleaning tasks and the recent resignation of the Nutritional Services Supervisor, who was responsible for overseeing kitchen operations. The facility's Dietary Infection Control Policy outlined the need for clean and sanitary conditions, but the observed practices did not align with these guidelines.
