Summary

The facility failed to maintain complete and accurately documented electronic health records for a resident, identified as Resident #5, who was observed wearing oxygen in his bedroom. Despite the resident's statement that he had been using oxygen almost daily for a couple of years and his care plan documenting oxygen therapy related to shortness of breath and a diagnosis of COPD, the most recent physician's order for oxygen had been discontinued in January 2023.
