The DON confirmed that the policy should be completed when a resident is transferred or discharged, but the Hospital-ER Transfer Form did not include a copy of the bed hold policy for Resident 1. The facility's policy requires that residents and their representatives be informed of the bed hold policy at the time of transfer, which was not adhered to in these cases.
