They failed to fully open the base of the lift, locked the rear casters, and experienced technical difficulties with the lift, which stopped mid-transfer. This improper use of the lift, including not following the manufacturer's instructions, further underscored the lack of competency among staff in handling mechanical lifts safely and effectively.
