Interviews with the Director of Staff Development (DSD) and Payroll Coordinator (PRC) confirmed that the actual hours were not completed, visibly posted, and signed by the DON within the required timeframe. The facility's policy, revised in August 2022, mandates that within two hours of the beginning of each shift, the number of licensed and unlicensed nursing personnel directly responsible for resident care must be posted in a clear and readable format. The policy also requires the charge nurse or designee to compute the number of direct staff and complete the Nurse Staffing Information form. However, the DSD and PRC failed to ensure the NHPPD forms were accurately completed and posted, resulting in residents not having access to the direct care daily staffing numbers. The DON stated that the NHPPD forms were reviewed and signed daily, but the deficiency indicates a lapse in adherence to the facility's policy and procedure.
