Failure to Implement Comprehensive RNA Care Plans

Greenfield Care Center Of Fillmore, LlcFillmore, California Survey Completed on 05-22-2024

Summary

The facility failed to implement comprehensive person-centered care plans for fourteen residents regarding the Restorative Nursing Assistant (RNA) program. The deficiency was identified through observation, interview, and record review, revealing that the RNA exercises were not provided as ordered for the residents. This failure was particularly evident for Resident 1, who reported that RNA exercises had not been provided since the RNA went on vacation. The review of Resident 1's RNA Flow Sheet confirmed the absence of documentation for the RNA services from 05/09/2024 to 05/21/2024, despite physician orders indicating the need for these services three times a week. Similar deficiencies were found for other residents, including Residents 4 through 16. Each resident had specific physician orders for RNA programs to maintain or improve their physical function, such as ambulation with a walker, assisted active range of motion (AAROM), and passive range of motion (PROM). However, the RNA Flow Sheets for these residents showed no evidence of the required RNA services being provided during the specified dates. For instance, Resident 4's RNA Flow Sheet lacked documentation of RNA services from 05/07/2024 to 05/21/2024, and Resident 5's RNA Flow Sheet showed no RNA services from 05/09/2024 to 05/21/2024. Interviews with facility staff, including the Director of Staff Development Assistant (DSDA) and the Director of Nurses (DON), confirmed the lack of RNA services and documentation. The DSDA mentioned that a Certified Nursing Assistant (CNA) trained as an RNA was reassigned to CNA duties on several dates, and no replacement was arranged to continue the RNA program. The facility's policies and procedures emphasized the importance of the Restorative Program in maintaining residents' optimal physical, mental, and psychological functioning, but these were not adhered to, leading to the identified deficiencies.

E
F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

Incomplete Care Plans for Hospice, Sensor Pad, and Oxygen Use: The facility failed to ensure care plans reflected key resident needs and behaviors for three residents. One resident’s plan did not include hospice services despite active hospice care and hospice aide visits. Another resident’s plan did not include use of a sensor pad even though he relied on it to call for help. A third resident’s plan did not reflect that he removed and reapplied his O2 cannula, although he stated he managed his O2 himself and an RN confirmed it.

F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

A resident with multiple fall risk factors and several recent falls had a care plan that listed floor mats, but staff did not have the mats in place until later and could not locate corresponding orders in the EMR. The resident also received multiple high-risk meds, including a benzo, antidepressant, gabapentin, and opioid, yet the care plan did not address medication-related fall risk despite the resident’s falls and the meds’ known side effects.

Missing PTSD Diagnosis and Interventions in Care Plan
D
F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

A resident with PTSD had the diagnosis listed in the face sheet, physician visit, and quarterly MDS, but the care plan did not include PTSD or related interventions. The DON stated she did not see PTSD in the care plan and noted it was important to include the diagnosis so staff could document interventions such as avoiding triggers. Facility policy required person-centered trauma-informed care and individualized interventions, preferences, and triggers to be documented in the care plan.

Failure to Include EBP in Resident Care Plans
E
F0656 F656: Develop and implement a complete care plan that meets all the resident's needs, with timetables and actions that can be measured.
Short Summary

The facility failed to include EBP instructions in the care plans of residents with a G-tube, a chronic wound, and an indwelling catheter. Observations showed PPE carts and signs directing staff to use gowns and gloves for direct care, but one care plan did not address EBP for G-tube or personal care, another lacked EBP guidance for wound care, and a third lacked EBP guidance for catheter care. The DON stated EBP should be care planned when required and staff were expected to follow the care plan and PPE guidance.
