The Director of Clinical Services confirmed that the Certified Nursing Assistant (CNA) responsible for R58's care on that day was an agency staff member who is no longer allowed to return to the facility due to neglecting their duties. The facility's failure to prevent the altercation between R160 and R117 and the neglect of R58 highlights significant deficiencies in the facility's ability to protect residents from abuse and neglect.
