The Evergreen Unit also had cleanliness issues, with a brown substance splattered on the wall near Resident 207's nightstand, which remained unaddressed over several days. Resident 407's room had blind slats lying on the floor, which were not removed despite being reported to the NHA and DON. Interviews with the NHA and DON revealed that they were aware of these issues and acknowledged that they should have been corrected. However, the observations indicated that the facility did not maintain a clean and homelike environment for its residents, leading to the identified deficiencies.
