Summary

The facility failed to revise resident care plans related to falls for three of seven sampled residents reviewed for abuse and neglect. Resident #1, who had diagnoses including dementia, anxiety, and heart failure, experienced eight falls between specific dates, three of which resulted in injuries such as skin tears and a fracture.
