Inaccurate MDS Assessment Due to Missing Weight Data

Summary

The facility failed to accurately complete a Minimum Data Set (MDS) assessment for a resident who experienced significant weight loss. Resident #6, who had a history of lymphedema, chronic non-pressure ulcers, depression, and hypotension, was admitted to the facility and weighed 179.7 pounds on 1/5/24. The Regional Director of Clinical Reimbursement and the Regional Director of Operations both confirmed that the facility typically uses the most recent weight closest to 30 days of the MDS's ARD and acknowledged the error in using the 1/5/24 weight for the 3/18/24 MDS. The failure to obtain and document a more recent weight for Resident #6 led to the inaccurate completion of the MDS assessment, highlighting a deficiency in the facility's assessment process.
