However, the medical record lacked documented evidence of an altercation between Resident #310 and Resident #311 prior to the incident on 7/3/2023. The incident was reported by Certified Nurse Aide #7, who witnessed the altercation and separated the residents. The Director of Nursing Services initially documented that the altercation was not abuse but later corrected this in the report to the New York State Department of Health, indicating that the incident resulted in abuse for Resident #311.
