Similarly, Resident #24 was transferred to the hospital for a stroke work-up, and there was no documentation that the bed hold policy was provided. Resident #4 experienced a sudden change in condition and was transferred to the emergency room, but again, there was no documentation that the bed hold policy was provided. During interviews, the Social Services Director (SSD) stated she was unaware that she needed to follow up to ensure bed hold notices were provided. The administrator confirmed that there was no documentation to show that bed hold notices were provided and stated that charge nurses were supposed to provide the bed hold policies, with the SSD following up to ensure completion. The facility's undated policy on Discharge/Transfer of Residents indicated that staff were to explain and give a copy of the bed hold form to the resident and/or representative, which was not adhered to in these cases.
