Interviews with facility staff revealed that the side rails were installed by hospice without proper follow-up or documentation. Resident #20, who had diagnoses including malignant neoplasm of the rectosigmoid junction, chronic kidney disease, type two diabetes, and high blood pressure, was also observed with half side rails on both sides of the bed in the upright position.
