Summary

The facility failed to ensure that a resident, identified as R16, was offered and wore a smoking apron to prevent accidents while smoking, as per her care plan. Interviews with R16 and other residents confirmed that she sometimes smoked without the apron. Staff members, including her assigned nurse and aide, were unaware of her need for a smoking apron. The facility's failure to ensure R16 wore a smoking apron and to educate staff on the necessary interventions for her safety led to the deficiency identified in the report.
