Despite this, the required documentation was not completed for the residents involved. The DON admitted that some staff had trouble using the PCC system and occasionally resorted to paper documentation, which was then scanned into PCC. However, this process was not consistently followed, leading to gaps in the required neurological checks and follow-up documentation. The failure to adhere to the facility's Post Fall Protocol resulted in incomplete assessments and documentation for the residents who experienced un-witnessed falls.
