Residents and staff who tested positive for RSV were not placed in appropriate isolation, and there was no outbreak investigation or contact tracing conducted. The facility's infection control practices were inadequate, as evidenced by the lack of training and proper implementation of transmission-based precautions. This resulted in multiple residents testing positive for RSV over several months without adequate measures to prevent further transmission.
