Summary

The facility failed to follow professional standards of nursing practice for three residents. For Residents #422 and #29, the facility did not measure the external length of the Peripherally Inserted Central Catheter (PICC) line as ordered by the physician. Despite the facility's policy requiring the measurement of the external central vascular access device with each dressing change, the measurements were not documented in the residents' medical records. Interviews with the nursing staff and the Director of Nursing (DON) confirmed that the measurements were not recorded, although they should have been according to the facility's policy and physician's orders. For Resident #24, the facility failed to identify and treat oral thrush, a fungal infection of the mouth. Despite the resident being observed with a thick, white coating on the tongue and lips, the condition was not reported to the Nurse Practitioner (NP) or adequately addressed. The resident's care plan included daily oral care, but the staff assumed that nothing could be done to treat the oral thrush. Interviews revealed that the NP was not aware of the current condition and had not been informed by the nursing staff, leading to a lack of appropriate intervention for the resident's oral thrush.
