Summary

Resident #86, who was admitted with a diagnosis of dementia and identified as hearing impaired, had a baseline care plan indicating the need for hearing aids. Despite this, the resident's clinical record lacked an impaired communication care plan, and there was no physician's order for the use of hearing aids.
